2010/09/07 Dennis Janda, Inc. Amendment 3 change to compensation, FY10 agreement AMENDMENT NO. 3 TO
PROFESSIONAL SERVICES AGREEMENT
THIS is Amendment No. 3 to that certain AGREEMENT for Professional Services
Agreement ("Agreement") made on March 12, 2010, by and between the City of Menifee ("City")
and Consultant: Dennis Janda Inc. ("Consultant"), which Amendment is made and entered into
on September 7, 2010 to increase the compensation as indicated below:
1. Section 2, "Compensation" is amended to read as follows:
Section 2. COMPENSATION. City hereby agrees to pay Consultant on fixed basis a
sum not to exceed $9,000 for services to be performed and reimbursable costs incurred
under this Amendment and as outlined on the attached Scope of Work, Option II. In the
event of a conflict between this Agreement and the Attached Scope of Work regarding
the amount of compensation, this Agreement shall prevail. City shall pay Consultant for
services rendered pursuant to this Agreement at the time and in the manner set forth
herein. The payments specified below shall be the only payments from City to
Consultant for services rendered pursuant to this Agreement. Consultant shall submit all
invoices to City in the manner specified herein. Except as specifically authorized in
advance by City, Consultant shall not bill City for duplicate services performed by more
than one person. The Parties have executed this Agreement as of September 7, 2010.
2. Attached to the Agreement is the attached two page document "SCOPE OF WORK —
Antelope and Watson Project. This is the Safe Route to School Project for Romoland
Elementary School which is being partially funded by a State of California Safe Route to
School Grant.

SCOPE OF WORK —Antelope and Watson Project
Option I
Survey:
I. Perform horizontal and vertical control survey for Topographic mapping.
2. Perform survey and locating existing centerlines and right of ways relative to
proposed sites.
3. Perform conventional ground survey locating aboveground improvements
including but not limited to utilities, pavement, curb and gutter, signs, medians,
etc. Also locate pavement breaks, i.e. fiowline, crowns, and change in material
(concrete, pavement, etc.)
Mapping:
1. Prepare engineering base map showing the centerlines and right of way together
with the topography and Improvements as discussed above at an accuracy of +!-
0.1' with at 1"=20' scale.
2. Prepare "Offer of Dedication" including legal description and plat for Right of Way
takes.
Delivery:
1. Provide (1) Hardcopy.
2. Provide (1) Digital Pile in AutoCAD 2007 format
All horizontal and vertical control to be consistent with establishment of
monuments and existing City facilities. The Horizontal Datum will be Tied to
California State Plane(NAD83) and the Vertical Datum to NAVD88.

Option II
Survey:
1. Perform horizontal and vertical control survey for Orthometric and Topographic
mapping
2. Perform survey and locating existing centerlines and right of ways relative to
proposed sites.
3. Perform conventional ground survey locating aboveground improvements
including but not limited to utilities, pavement, curb and gutter, signs, medians,
etc. Also locate pavement breaks, i.e. flowline, crowns, and change in material
(concrete, pavement, etc.)
4. Perform aerial photogrammetry for 150' strip mapping using photogrammetric
methods at a scale of 1" = 20' with 1' contour intervals to meet or exceed NMAS
and ASPRS Class 1 accuracies.
Mapping:
1. Prepare engineering base map showing the centerlines and right of way together
with the topography and improvements as discussed above at an accuracy of -�/-
0.1' with at 1"=20' scale.
2. Orthometric mapping overlay at a scale of 1" = 20' with V contour intervals to
meet or exceed LAMAS and ASPRS Class 1 accuracies.
3. Prepare "Offer of Dedication" including legal description and plat for Right of Way
takes.
Delivery:
1. Provide (1) Hardcopy.
2. Provide (1) Digital File in AutoCAD 2007 format
