Regular maintenance per the maintenance checklist 3 . Service report upon completion of the maintenance and inspection check. 4 . Additional services as may be included in the attachments to this agreement. The service outlined in this agreement will be performed during normal working hours: The services included in this contract will be performed for the sum of $300.00 Repair service requested by the customer will be provided on a preferential basis at the rate of $ 70.00 per hour, with a 15% discount on parts and labor. No repairs or improvements will be performed without customer approval. A comprehensive precision tune up and professional cleaning with filters replaced will be performed each year during the months of: JAN FEB MARCH APRIL MAY JUNE JULY AUG SEPT OCT NOV DEC We agree to perform routine maintenance and inspection service as indicated for a period of one yea from the date cf this agreement- The agreement remains in force unt�yea by either party with a 60 - day written notice but subject o price revision after the anniversary date. To ensure completeness we follow this checklist: 1. Check Blower System 5. Check Control Panel in Unit A. belts/pulleys/bearings 6. Check Condensate Drains B. lubricate as needed 7. Check Heating System (in season) 2- Check Evaporator System A pilot A. coil & components B. gas valve 3. Check Condenser System C. ignition system A. coils/fans & motors 8. Change Filters in Units B. lubricates and cleans as needed 9. Add refrigerant if needed for additional cost. 4.
