COMMERCIAL PRIORITY CUSTOMER SERVICE AGREEMENT

The routine maintenance and inspection service to be performed as a part of this
agreement includes:
1 . 4 Inspections per year on a scheduled basis by a qualified
Technician.
2 . Regular maintenance per the maintenance checklist
3 . Service report upon completion of the maintenance and inspection
check.
4 . Additional services as may be included in the attachments to this
agreement.
The service outlined in this agreement will be performed during normal working
hours: The services included in this contract will be performed for the sum of
$300.00 Repair service requested by the customer will be provided on a
preferential basis at the rate of $ 70.00 per hour, with a 15% discount on parts
and labor. No repairs or improvements will be performed without customer
approval.
A comprehensive precision tune up and professional cleaning with filters
replaced will be performed each year during the months of:
JAN FEB MARCH APRIL MAY JUNE JULY AUG SEPT OCT NOV DEC
We agree to perform routine maintenance and inspection service as
indicated for a period of one yea from the date cf this agreement- The
agreement remains in force unt�yea
by either party with a 60 - day
written notice but subject o price revision after the anniversary date.

ROUTINE MAINTENANCE CHECKLIST
