Social Worker Action Group (SWAG) continues working within the City of
Menifee every Wednesday and Friday. Working jointly with the Crime
Prevention Officer and Park Rangers they are making contact with
homeless in the city and assisting them with getting vital documents,
housing vouchers, medical assistance, shelter, etc. They continue to case
manage many of the chronically homeless in the area by seeking creative
solutions to each individual’s needs.

Closing the Gap to Housing
In the month of August SWAG was notified that two of their homeless
clients in Menifee have been approved for housing programs. Utilizing
the Service Prioritization Decision Assistance Tool (SPDAT), information
taken by SWAG during initial interviews is entered into a standardized
system which prioritizes individual needs across the county. Both of
these clients scored highly and in time made their way to the top of the
list for housing. This is a monumental step towards getting these clients
permanently off the street and it becomes critical to ensure a smooth
transition into housing. The entire outreach team has dedicated extra
time and resources towards assisting these individuals in gathering the
required documentation and conducting follow up interviews to ensure
nothing falls through the cracks. SWAG continues to provide results
which are making a change in our community.
Coordinated City Efforts
When Park Rangers became aware that chronic violators of the City's
Solicitation Ordinance were staying on a property where Code
Enforcement was also working a case a quick response by multiple City of
Menifee resources swung into action. Ongoing problems at the property
had led to substandard living conditions which had to be addressed by
Code Enforcement. With assistance from the Crime Prevention Officer
and Park Ranger, SWAG was put into contact with the individuals on the
property early in the process. When the property was ordered to be
vacated, Animal Friends of the Valley was contacted to assist in collecting
animals which could no longer be cared for. An elderly couple on the
property was identified as needing further assistance and SWAG worked
to find emergency housing and contact other available resources.
Through each department taking on a role the property was cleared of
trespassers and the City is working with the property owner to get into
compliance. SWAG will continue to provide case work for the individuals
involved and will be looking to find a housing solution.
